What a semaglutide trial actually found about biological aging
A post hoc analysis found slower movement across several epigenetic clocks in adults with HIV-associated lipohypertrophy—but it did not test lifespan or prove an anti-aging effect.

A peer-reviewed Nature Communications paper reports that semaglutide changed several DNA-methylation measures of biological aging over 32 weeks. The result is interesting, but it does not show that the drug extends human lifespan or broadly reverses aging.
What the researchers studied
This was a post hoc exploratory analysis of stored blood samples from a randomized, double-blind, placebo-controlled phase 2b trial in adults with HIV-associated lipohypertrophy. The parent ClinicalTrials.gov record focused on visceral fat and cardiometabolic outcomes; epigenetic aging was not a prespecified endpoint. The analysis included paired samples from 45 semaglutide participants and 39 placebo participants.
What the analysis found
- Semaglutide was associated with slower change on several second- and third-generation epigenetic clocks relative to placebo.
- DunedinPACE was 0.09 units lower, which the authors described as an approximately 9% slower pace of biological aging.
- Not every clock changed significantly, and the reported P values were not adjusted for multiple comparisons.
Epigenetic clocks estimate biological-age-related patterns from DNA methylation. They are biomarkers, not direct measurements of lifespan, disability-free survival, or clinical rejuvenation.
The limits that matter
- The aging analysis was exploratory and not prespecified in the parent trial.
- The sample was modest and limited to adults with HIV-associated lipohypertrophy, so the result may not generalize to other populations.
- Follow-up lasted 32 weeks, too short to establish durable clinical aging outcomes.
- Biomarker movement cannot by itself establish longer life or better long-term function.
The defensible takeaway is narrow: semaglutide altered several epigenetic aging measures in this trial sample. It is a reason for targeted follow-up research, not a reason to use semaglutide for longevity outside an approved clinical indication.
